FORMS FOR ADMISSION:

Application:  The free and easy application including personal, financial and medical information is a great first step in getting forms and paperwork completed for admission.  Those considering assisted living or healthcare will need to come in for an interview with a Martha Franks Medical Services Nurse before the application can be processed.  Click to download form.

Residence Reservation:  If there is a certain cottage, patio home, apartment or dormitory room that you want to reserve, please request a reservation form to pay a small deposit and hold your choice for an agreed number of days, giving you time to make preparations for your move. Guarantee of Payment:  If a family member, church or other source wants to contribute funds towards a person's residential living, this form may be used.  Also, family members who want to guarantee the payment of their loved one's account to aid in the admission process may use this form. Forms may be downloaded or requested by mail, email or telephone.  Completed forms may be mailed to the attention of Lisa Yarber/Director of Marketing at the Martha Franks address, faxed to 864-681-8284, or dropped off at the marketing or healthcare admission offices.
